Given that the "crack in time" was created by the Time Lords, and the Eleventh Doctor encountered the crack well before the events of "The Day of the Doctor", likely the latter.

It is impossible to know. It depends on the viewers interpretation of time. Some people believe that time can be changed, some people believe that time always unfolds in a certain way, and some people believe that it depends on the vantage point.
